In-person:

• All owners must be present with a valid government-issued identification

 

By mail:

• All owners must complete an Affidavit of Loss form and have it notarized

• Certificate of Title application is recommended as well but not required

• Check, Money Order or Cashier’s Check payable to D.O.L for $35.50

 

Important information about titles:

• All documents must be originals and must not contain any alterations. Any alterations may void the document and a new document or additional documents may be required.

• Not all transactions are the same. We ask that you keep in mind that you may be required to provide additional paperwork.

• If you need an exact amount of what it will cost, please contact our office for instructions, we can usually provide you with a total same day.
